Community Giving

Answer the following questions to see if your organization is eligible for a community giving grant.

The SCAN Health Plan Community Giving program provides one-time 12 month grants to nonprofits and government agencies that provide direct services to older adults (age 55+) and caregivers within SCAN Health Plan service areas. Our funding priorities include nutrition access and food security, emergency financial assistance and other projects aligned with the goal of keeping older adults healthy and independent. Our application process is by invitation only with grant applications reviewed quarterly.

Grant eligibility checklist:

1. Does your organization align with SCAN Health Plan’s Community Giving mission?

Our mission is to continue to find innovative ways to enhance older adults' ability to manage their health and to continue to control where and how they live – inclusive of age, disability, race, ethnicity, language, gender identification, sexual orientation and other demographic characteristics.

2. Does your organization serve older adults (age 55+) and caregivers in one or more SCAN Health Plan service areas?

The following counties are covered by SCAN:

  • California: Alameda County, Fresno County, Los Angeles County, Madera County, Orange County, Riverside County, San Bernardino County, San Diego County, San Francisco County, San Mateo County, Santa Clara County, Stanislaus County, Ventura County
  • Texas: Harris County, Bexar County
  • Nevada: Clark County, Nye County
  • Arizona: Maricopa County, Pima County, Pinal County
  • New Mexico: Bernalillo County, Sandoval County

3. Is your organization classified as a tax‐exempt organization under section 501(c)(3) of the U.S. Internal Revenue code or a governmental entity?

To be eligible for funding, organizations must legally operate as one of the following and meet these basic criteria:

  1. Tax exempt under IRS code 501(c)(3) or a government entity in good financial standing. Financial health will be assessed based on the IRS Form 990 or 990-EZ annual return. Annual revenue must be greater than $50,000 as indicated in the most recent annual return form. Priority will be given to those that have filed three or more years of IRS 990 Form 990 and/or 990-EZ annual returns.
  2. Under the formal fiscal sponsorship of another organization’s 501(c)(3) status for at least three years and in sound financial condition.
  3. A program of a local government entity or an un-endowed foundation established to support a public entity, (e.g., Friends of the Commission on Aging).

As part of administrative and due diligence review, additional information may be requested including: most recent audited financial statements, annual operating budget, detailed program budget, sources of other income, proof of tax-exempt status, articles of incorporation and current list of board of directors.

4. Has your organization been in existence for at least three (3) years?

SCAN Health Plan Community Giving program supports nonprofits and government agencies that have been operating at least three years with established programs serving and directly impacting older adults and their caregivers. SCAN requires prospective organizations to demonstrate a strong operational history and alignment with key funding priorities, such as nutrition access, emergency financial assistance and aging in place supports. SCAN also prioritizes direct service organizations with proven capacity, community reach and demonstrated ties to the older adult and caregiver community they serve.

Preference will be given to organizations with diverse funding sources focused on older adults, particularly those that have some existing funding for their programs and are seeking a grant to cover specific funding gaps.

5. Does your organization have a history of providing direct services to older adults?

SCAN Health Plan Community Giving program supports programs and services that address the needs of older adults in areas such as nutrition access, emergency financial assistance and other projects aligned with the goal of keeping older adults healthy and independent. Organizations must have targeted projects or a strategic focus on serving and positively impacting the lives of older adults and supporting caregivers. Funding is available to support programs specifically serving older adults or caregivers within SCAN Health Plan service areas.
