Caregivers & Older Adults Connected & Healthy
COACH provides customized support to people 55+ and their caregivers. COACH connects program participants to local resources and services, based on individual assessment of needs and geographic location. COACH provides services at no cost to the community, including those who reside in the greater Inland Empire, Long Beach, Los Angeles County or Orange County areas. Case management services may include:
- Phone calls, video chat and home visits as needed
- Comprehensive evaluations, care planning, service coordination and advocacy
- Providing resources or referrals to help with personal care, daily living activities and household tasks
- Connection to transportation and meal assistance
- Fall prevention assessment and training
- Referrals to services for family caregivers
- Connection to pharmacist services and medication management
- Health education and information
- Life transition and planning
- Stress management and prevention
- Help navigating Medi-Cal and Medicare benefits
